Umpires and technical officials interested in participating in the 2012 National Championships are asked to fill out the following form, which will be submitted to Field Hockey Canada for review.  

The deadline to apply is March 30th, 2012.  

* Officials travelling from out of province to the tournament by air will be provided a subsidy of $300. Officials travelling within the province who are not from the host city may be eligible for a small travel subsidy.  For any questions, please contact Kelly Michael at fhc@fieldhockey.ca.
